Yep, this was taken from episode 4 after Setsuna's mind!ravage of the park he was in and his meeting of Saji and Louise.

Remember that part where Saji tries to remember Setsuna's name while introducing him to Louise? He hesitates at first right? XD

Some fanartists have taken to putting in different names ROFL. So, in that 4koma that I linked to, he introduced Setsuna as Yagami Raito [Because Setsuna's seiyuu voices Death Note's Yagami Raito as well].

There's another one from another site where Saji introduces Setsuna as Fujiko F. Fujio whut. XDDD";;;
